At CollabCare Group, we are dedicated to creating an all-inclusive, compassionate, and empowering environment for individuals in need. Our private independent shared-living housing program is designed to support those who are homeless, displaced, facing addiction, neuro-divergent, survivors of trauma, and the aged by offering a safe and supportive space for healing, growth, and community.

We believe in the inherent worth and dignity of every person, regardless of their age, race, color, gender identity, sexuality, familial status, national origin, or disability. Our approach centers on providing tailored support and resources that address the unique needs of each individual, fostering a sense of belonging and opportunity for all.

 Safe Space To Rest

A comfortable bed provided for each housing participant to ensure a safe space and a good night's rest. Parking spaces are available at the residence and on the street in front of the residence.

 All Utilities Paid including Wi-Fi

No need to worry about utilities—electricity, water, and other essential bills are covered. Free Wi-Fi access to complimentary high-speed internet, ensuring connectivity for work, education, or leisure

 Continental Breakfast

Daily access to a convenient and nutritious continental breakfast to start the day. A full kitchen is available for our participants to prepare meals and store food.

 Community Therapy

Regular weekly group therapy sessions to support mental health, personal growth, and community building

 Security System

Cameras are installed in various areas and digital door locks are installed on each bedroom door to provide added protection in your safe space

 Laundry

Washer and Dryer available within the residence for ease and convenience.

 Engaging Activities

A variety of scheduled activities designed to promote social interaction, skill development, and fun.

 Various Payment Options

Participants may pay program costs via Zelle, Cash App, Certified Check, ACH Draft or Cash. We do not accept medical insurance, Medicare or Medicaid.

All participants must demonstrate a certain level of self-sufficiency and autonomy in daily activities. The following requirements must be met for admission:

Atomic Independence 
Participants must be atomically independent, meaning they possess the physical, cognitive, and emotional abilities to complete daily tasks independently without external assistance. 

Required Abilities: 
  • Ability to independently perform personal care tasks such as bathing, dressing, and grooming. 
  • Capability to prepare meals and manage basic cooking tasks. 
  • Ability to carry out routine household chores, such as cleaning, laundry, and maintaining personal living spaces. 
Mobility
Participants ust be able to walk independently or use mobility aids (such as a walker or cane) without requiring continuous assistance. Individuals who rely on a wheelchair or other mobility devices must be able to manage their mobility and navigate the environment without assistance. 

Work and Employment 
Participants must have the ability to engage in employment or productive activities, whether in the form of a paid job, volunteering, or other meaningful engagements. The ability to maintain a work schedule and manage the demands of employment or daily activities independently is essential. 

Social and Behavioral Expectations 
Participants must demonstrate appropriate social behavior in shared living environments, including the ability to communicate effectively and respectfully with housemates and staff. Emotional self-regulation and conflict resolution skills are required to ensure a harmonious living space. 

Health and Safety 
Participants must be able to independently manage their health and safety needs, including daily tasks, making and following up on medical appointments, medication coordination and intake. some assistance maybe available according to the need.

No. We do not accept medical insurance,  Medicare or Medicaid.  We are a private independent shared-living housing program. We are cash based. We accept various payment options such as Zelle, Cash App,  Certified Check,  ACH Draft and Cash. Personal Checks are not accepted.
No. We do not offer transportation.  However, we can assist with finding transportation for our participants.
